In addition to providing expert neurological care, we have provided helpful information, forms and questionnaires to help you prepare for and make the most of your visit to our practice.

We would like to provide you with targeted, personalised and efficient care right from the start. To help us do this, it would be very helpful if you could bring the following documents with you or upload them in advance:

Health card, identity card or passport
Completed medical history questionnaire
If necessary, a referral with a specific query
Existing medical reports / findings regarding previous treatment or diagnostic imaging (MRI, CT, X-ray, hospitalisation)
A current list of your medications from your GP
Current blood test results
Reading glasses and hearing aids, if required
If applicable, headache diary/pain questionnaire

Coverage of costs

This is a private practice. Treatment can be billed to private health insurance providers or paid for by patients themselves. If you are covered by the state health insurance scheme, you can ask your health insurance provider about reimbursement options.

Of course, patients with statutory health insurance are also welcome to use our services as private patients – please do get in touch.

Medical certificates

Fees apply for medical certificates, which must be paid in advance. These costs vary depending on the scope and level of detail of the certificates and are to be borne by the patient.
